This form is for parents and caregivers who share custody and would like school bus transportation from more than one home address. It collects details for both households, including eligibility, contact information, and a required Joint/Shared Custody Transportation Safety Plan (JSCTS Plan). Parents must outline how their child will know which bus to take, as well as a safety plan if the student boards the wrong bus or gets off at the wrong stop. The form must be signed by both parents/caregivers and approved by the school and board each year.
